Health Insurance & Payment
I am in network for Premera Blue Cross, Lifewise and most Blue Cross plans.
Beginning on July 30, 2024, I will no longer be a participating provider for Regence BlueShield (which includes Uniform Medical Plan and the Federal Employees Plan).
I am out of network for all other insurance plans.
Payment by check is preferred, and I do not currently accept credit or debit cards, including those from HSA or FSA accounts. If you have such an account, please check if you can obtain checks for the account.
Payment should be made out to my name and mailed to my office address (please include the suite #435).

- If you have insurance for which I am not in network, please consult your insurer to determine what your out-of-network coverage is. Out-of-network care may include a high deductible, and the allowable amount (what insurance deems is an appropriate charge for a type of service) may be lower than my charged rate.
